Ricardo,   The use of Patient Portals, such as My Chart , which I have experience with either a professional or a user, had a positive impact on the patient’s relationship with the Health System.           Patients had more efficient and faster access to their information, in addition to increasing the interrelationship with the provider, whether to clarify doubts, request refills, make appointments, have access to diagnoses, fill out forms, update their insurance data in order to self-monitor your health and data (Vorvick, L. J. , 2022)          Portal patients, such as My Chart, made patients administrators of their own health, because in addition to the information and access available, these portals brought medical and educational information to improve the quality of life and prevent diseases, consequently reducing the educational re-work carried out previously by providers, making the whole process more efficient. (Hoover, R. , 2017)         Recent research points out that Patients Portals, enhance the doctor-patient relationship, will also improve adherence to therapies, since the process has become faster. (Carini, Elettra, et al. , 2021)         But not all impacts were positive, many patients have concerns about the use of their data by these portals, however safe they may seem, creating a barrier against this tool. Another major barrier against this system is the lack of technological knowledge of many patients that prevent access, mainly related to elderly people.(Carini, Elettra, et al. , 2021) Information system and nursing practice today          From the point of view of nursing, within what we saw this week and in my opinion, what most shaped the state of nursing practice today were the Electronic Health Records.        Despite being considered by many nurses as a time-consuming tool, after data is entered, Electronic Health Records make the process quick and efficient.        Nursing continuously benefits from this situation, as it has quick access to the patient’s history, which is reflected in its next actions for the patient, associated with this, it reduces errors and the number of lawsuits, either against the professional himself or against the institution to which he works. work, in addition, access to information and data favors the construction of critical thinking and decision-making by nurses (HabibiKoolaee, M et al; 2015)        The fact is that as this technology evolves and records are made available, all stakeholders benefit from the process and for Nursing this was a great advance since it made work more efficient and safer for the professional.

Laurell, The Impact of Electronic Health Records on Nurses                Electronic health records (EHR) have had a major impact on the delivery of care for nurses. EHRs makes patient charts more accessible, trends more visible, and medication administration safer. The use of paper charts where slow in delivering care because nurses had to go through and decipher orders. Also, if there where any complications they had to wait for an order from a doctor to be written or it was given verbally and errors occurred. Today with the EHRs providers can enter orders into patient charts remotely. If a new medication is being implemented the order from the Dr. is verified from the pharmacist first then the nurse can administer the medication. The EHR have safety features that makes medication administration safer through scanning the patient and going through the six rights of medication administration. An EHR system that was recently implemented in a community hospital in Vermont “reported  60% decrease in near-missed medication events (The Office of the National Coordinator for Health Information Technology, 2019).” which information system do you think has most shaped the state of nursing practice today, and why?                 I think Telehealth has made a recent impact on nursing practice due to the recent COVID 19 pandemic. With outpatient offices being closed providers where able to continue care through telehealth. Telehealth allowed providers to visibly see their patients through video conference calls. Medicare telehealth policies changed during the pandemic impacting Medicare clinician services, Medicare hospital outpatient services, and home health agencies (Health Resources and Services Administration, 2023). The policy changes will continue to transform after the pandemic and lead to more patients utilizing telehealth services. “Nurses and nurse practitioners are able to use telehealth for remote monitoring, patient education and consultation (Neville, 2018).
